Inclusion criteria:
- Diabetes 2 for more than one year
Home monitoring

App (Electronic chart system)
- Messaging
Intervention: Patients oploaded data to online server. Nurses monitored the patient system, reviewed patient information, and sent optimized recommendations to patients via the device’s electronic chart system (every week for 3 months, every second week for 3 months). Patients visited outpatient clinic every three months.
