Intervention: Patients upload clinical data to webpage. Health provider view information and conducted recommendations for patients via internet or SMS. Recommendations consisted of continuous education on diet, exercise, medication adjustment, and bodyweight levels. All medication adjustments were communicated to the patients’ doctor.
