Intervention: Individualized telerehabilitation program to address lifestyle changes and improve self-care (Teledialog project). Patients were able to monitor their data at all times. Staff monitored the values twice a week and had contact with the participant if needed. Communication between healthcare staff, patients, and partners. Each patient received guidance in physical training, education in lifestyle changes
